My minor is healthy and physically fit to participate in a tour of the difficulty indicated above.
My minor has the required equipment and clothing as described for the tour.
I authorize the Guide to give my minor first aid, to call the emergency services (TOPR / Horská záchranná služba), and to pass on to them information about my minor's health. I acknowledge that decisions on medical procedures and treatment are made by medical or rescue personnel. I will be reachable at the phone number given above during the tour.
I give my explicit consent to the processing of the information about my minor's health provided in this document (Art. 9(2)(a) GDPR), solely to ensure their safety during the tour. Providing this information is voluntary; I may withdraw the consent at any time (kontakt@tatractive.pl), which does not affect the lawfulness of prior processing.
I consent to the processing of my and my minor's personal data in line with the privacy policy.
